  This form must be submitted with your application and must be on file prior to making campaign expenditures or accepting campaign contributions. If you do not intend to accept more than $500 in political contributions, or make more than $500 in political expenditures, please complete and submit FORM CTA PG2 - Candidate Modified Reporting Declaration. If during your campaign, you are about to exceed $500, you will need to file FORM CTA PG1 and contribution reports as outlined under FORM C/OH Instruction Guide.
